In order for every student to gain the most from his/her CISB experience we kindly ask you to complete the new ‘annual’ health questionnaire on all CISB students for whom you care. (This questionnaire is not to be confused with the admissions health history form which some of you have completed).

Your time in this matter is very much appreciated and will ensure every student’s health needs are better meet.
